Free or Low-Cost Mammogram or Treatment

While Komen Central and Northern Arizona (Komen CAN AZ) is not a direct service provider of mammograms or breast cancer treatment, we are committed to providing our community with access to these vital resources.

Well Woman HealthCheck Program
Arizona’s Well Woman HealthCheck Program, a state-wide program that provides free cancer screening to those that qualify, is the first contact for women with no health insurance and no diagnosis of breast cancer. The program is operated by the Arizona Department of Health Services through a cooperative agreement with the Centers for Disease Control and Prevention. Women on the program may receive a Clinical Breast Exam, Mammogram, Pelvic Exam and Pap Test. To qualify, call:

Breast Health for Life: Ensuring Low-Income Uninsured Patients Access to Screening
Mountain Park Health Center's BREAST HEALTH FOR LIFE program will use Komen Central and Northern Arizona grant funds to provide 450 screening mammograms, 100 diagnostic mammograms and 100 breast ultrasounds to uninsured, low-income patients.  A fulltime, bilingual Breast Cancer Screening Patient Navigator will assist patients throughout the breast cancer screening process and link patients in need of advanced diagnostics with timely, affordable care.

Breast Health Screening for North Phoenix’s Uninsured and Underinsured
Desert Mission serves low-income populations, primarily in North Phoenix, with high quality primary health care. With support from the Komen Central and Northern Arizona for the Cure, Desert Mission will educate more than 500 women on breast self-awareness, refer 160 women for screening mammograms, and help 20 women receive diagnostic procedures when necessary. Transportation assistance in the form of bus passes is available to those in need.

Breast Screening and Diagnostic Services
The Community Health Center of Yavapai (CHCY) is the major health care provider in Yavapai County for low-income, uninsured, and underserved individuals, especially in the Hispanic community. Purpose of the Program:  To increase access to direct care by removing the cost barrier for breast cancer screening/diagnosis when no other funding source is available, and promote continuum of care by providing ongoing primary care services. Key Activities: Provide, free of charge: 200 screening mammograms, 100 diagnostic tests (diagnostic mammograms/ultrasounds), up to 6 ultrasound guided needle biopsies and 104 hours of Patient Navigator services in English and Spanish.  If diagnosed, the patient receives assistance to secure treatment including applying for the Breast and Cervical Cancer Treatment Program (BCCTP).

CGRMC Foundation Screening and Diagnostic Mammography Program
The CGRMC Foundation Screening and Diagnostic Mammography Program will provide screening mammograms for 150 medically underserved women who are uninsured or underinsured, ages 40 and older, living in Pinal County. The target populations will include Hispanic, African American and White women lacking in these financial resources. The program also includes screening or diagnostic mammograms for women under the age of 40 or men who are identified by a physician as medically at risk due to family history or medical concerns.

Gila River Indian Community Screening and Diagnostic Mammography
In order to combat barriers associated with screening of Native American women, who are uninsured and insured; Gila River Health Care has committed resources to providing a Breast Cancer Continuum of Care Model through the Care Coordination Cancer Awareness Advocacy Navigation (“CAAN”) Program in partnership with the Radiology Department who will administer the SGK for the Cure ® Screening and Diagnostic Program. The goals of the project are to identify, contact and screen 110 Native American women who are either underinsured or uninsured that don't qualify for either AHCCS or WWHP, as well as provide 30 diagnostic mammograms and 30 breast ultrasounds and increase awareness among Native American women of BSA and shift the focus from solely BSE.

Golden Gate Community Center- Fighting Breast Cancer Together
Golden Gate Community Center will provide free screening mammograms to women from West central Phoenix neighborhoods. Women living in this area are predominantly uninsured, Hispanic, and low-income. The purpose of the program is to increase screening rates and breast health education among this underserved population.  Over the course of the grant year we propose the following objectives: Objective 1: Provide screening mammograms to 200 medically underserved women during project year through 5 screening and health events held at Golden Gate Community Center; Objective 2: Provide case management and referral for follow-up services to a medical provider for 100% of the women who receive abnormal mammogram results during the project year; Objective 3: Provide educational information through breast health talks and community outreach to 500 community women.

La Sagrada Health Fair
La Sagrada Health Fair is designed to provide 80 mammograms and additional health services to the residents of a small neighborhood located in North Phoenix  Over 92% of the families in this neighborhood live below the poverty level. This one-day health fair will bring services to the families that are unable to obtain otherwise due to being under-insured or uninsured. The health fair, includes providers who are willing do health screenings, that include blood sugar checks, cholesterol checks, vision and hearing checks, BP checks, and BMI counseling.

Screening and Advanced Diagnostics
Maricopa Health Foundation and Maricopa Integrated Health System (MIHS), Maricopa County’s only public hospital and healthcare system, will partner with District Medical Group, and the Well Woman Healthcheck Program to leverage funds to provide screening mammography and advanced diagnostics to the medically underserved (Hispanics, African Americans, Native Americans, refugees, the low-income, the undocumented and the uninsured) in Maricopa County. Key activities will provide 150 screening mammography’s and 76 advanced diagnostic services to women who are ethnically diverse and low-income.

Screening and Diagnostics at St. Joseph’s Hospital and Medical Center
St. Joseph’s Hospital and Medical Center’s (SJHMC) screening project aims to save lives by providing screening mammograms to detect breast cancer.  Through this program, SJHMC aims to screen 300 uninsured and underinsured women living in Maricopa County, as well as provide all necessary follow-up diagnostic and biopsy services. A bilingual patient navigator will guide patients through the system to ensure continuity of care. The program will be evaluated based on number of patients who utilize free screening mammography services who otherwise would not have been able to afford a mammogram and who do not otherwise qualify for assistance or programs.

The Reaching Out Program
This project will address breast health needs of underserved and uninsured women in metropolitan Phoenix. The program will provide education, community outreach, screening exams, diagnostic services, breast cancer treatment, and survivor support services in a caring and culturally sensitive environment.  The program will employ Nurse Navigators to assist patients throughout the breast cancer journey. Breast health educational materials at events and in the Breast Center to 1500 newly diagnosed patients. 40 individuals will receive screening. Provide diagnostic services to at least 50 Program patients requiring additional testing as well as diagnostic services. Provide a full spectrum of breast cancer treatment and survivorship to at least 3 new patients and 11 current patients

 TREATMENT PROGRAMS

Breast and Cervical Cancer Treatment Program (BCCTP) for Breast and Cervical Cancer TREATMENT
• Be diagnosed as having breast cancer, cervical cancer, or pre-cancerous cervical lesions
• Be under age 65
• Provide a verifiable Social Security Number
• Be a resident of Arizona
• Be a U.S. Citizen or a non-citizen qualified to receive Medicaid
• Meet the financial eligibility requirements for the WWHP
• Be uninsured or have insurance that does not cover the treatment of breast and/or cervical cancer. You may also qualify if you are insured but your insurance excludes cancer treatment as a pre-existing condition, OR
• You have exhausted the lifetime limit for all benefits under your insurance plan including breast or cervical cancer treatment
• You must not be eligible for AHCCCS Health Insurance under any other Medicaid group ◦If you are eligible for AHCCCS Health Insurance under another group, you must be approved under that program.

Breast Cancer Treatment for Low-Income Women
Maricopa Health Foundation and Maricopa Integrated Health System, Maricopa County’s only public hospital and healthcare system, will partner with District Medical Group, and Integrated Medical Services (IMS) to leverage funds to provide breast cancer treatment to the medically underserved (Hispanics, African Americans, Native Americans, refugees, the low-income, the undocumented and the uninsured) in Maricopa County. Key activities will provide 22 women with surgical procedures (i.e., lumpectomy, mastectomy) and 21 women with radiation treatment.

Therapy Services/Breast Cancer Management
This grant will help fill unmet needs by providing comprehensive lymphedema, management and massage therapy to people without the means to pay fully for treatment.  Many of our patients live far away from our clinics and do not have the funds to pay for fuel.  They may need to stay overnight.  The travel and lodging funds will help those patients attend treatment sessions. Evaluation will be of numbers served. We anticipate that 40 people will need lymphedema treatment assistance, 75 breast cancer patients will benefit from funds for massage therapy, and 18 people living in remote areas, will need help with expenses and lodging.

Patient Care Cost Assistance
“The Patient Care Cost Assistance Project” of Northern Arizona, funded by Komen Central and Northern Arizona, provides financial assistance for breast cancer diagnoses and treatment. This project provides a safety net for underserved populations in Coconino, Navajo, Mohave, Yavapai and Apache Counties including Hispanic, Native American, uninsured and under-insured individuals. This program successfully linked women to timely, affordable care and established a treatment network of providers accepting reduced rates.  This project provides direct assistance to 90+ women and enables continued outreach to the undocumented population, resulting in 300+ mammograms annually.
