Application Guidelines

The Phoenix Affiliate Community Grants are one-year grants for community projects providing outreach and education, screening services, and treatment services to the medically underserved, uninsured or underinsured.

Deadline for Application
Applications must be postmarked on or before Friday, December 17, 2010 or received in person by 5:00 p.m. Friday, December 17, 2010.  Faxed applications will not be accepted.  Please mail to:

Grant Categories
An organization may apply for a grant in the individual or combination grant category; not both.

Grant Categories - Individual
Education projects will be funded up to $50,000.  We are looking for education projects that are effective in promoting behavioral change or empowering individuals to make good breast health decisions. Organizations requesting funding for educational projects should include only interventions that are evidence based and can demonstrate a successful outcome/result. 

Screening projects will be funded up to $75,000.  Screening grants should use Komen funding as a “last resort” for those women who are medically underserved, uninsured and/or underinsured.

Treatment projects will be funded up to $150,000. All Komen funding for treatment service projects must be used to serve medically underserved, uninsured, and/or underinsured populations.

Grant Categories - Combination
This category has been added for organizations that have developed a program that encompasses a combination of the above grant funding categories and to simplify the grant application process for potential grant recipients. Grant applications maybe written for the following combination of grant categories:

Education and Screening
     Education up to $50,000; Screening up to $75,000 for a total not to exceed $125,000

Screening and Treatment
      Screening up to $75,000; Treatment up to $150,000 for a total not to exceed $225,000

Education, Screening and Treatment
      Education up to $50,000; Screening up to $75,000; Treatment up to $150,000 for a total not to exceed $275,000

Statement of Need
We are seeking projects that focus on education and outreach, screening or treatment services among medically underserved, uninsured or underinsured communities. Please refer to our 2009 Community Profile for more in-depth information.

Pre-application Workshop
We strongly encourage all applicants to attend a Pre-Application Workshop before applying for a grant from the Phoenix Affiliate.  The workshop will be held on Tuesday, September 14, 2010 from 9:00 a.m. to 11:00 a.m. at the Flinn Foundation 1802 N. Central Avenue, Phoenix, AZ 85004. Map to Flinn Foundation.

Grant Application Packet

There are two pieces of the Community Grant application packet.  The first piece is the 2011-2012 Community  Grant Program Granting Guidelines and the other is the Community Grant Program Application for Funding Packet.
