The 6-month progress report is due Oc tober 30, 2009.  Please note if you have spent the first half of your grant funds, you may send in the progress report before October 30.  6-month 2009/2010 progress report

CHANGES DURING THE GRANT YEAR

We realize from time to time that conditions change during the grant year.  As specified in the grant contract, you need to receive approval from Susan G. Komen for the Cure before implementing any changes to the grant.

To request a change for one of the following circumstances, please complete a Change in Grant Request Form.

      Change of Grant Start Date
      No-cost Extension (change in ending date only)
      Budget Change (also complete the Budget Change request form)
      Personnel Change
