Every two years, each Komen affiliate conducts a community needs assessment, known as the Community Profile Report, to identify populations most in need of breast services and to identify gaps in services that must be addressed to improve breast health in our community.
The Community Profile Report is used initially to identify the gaps between services that are currently provided and services that are needed.
The Komen Phoenix Affiliate utilizes these priorities to provide the framework for our operations and drive our organization. Our Community Grants program uses the priorities in the Community Profile to identify organizations that will receive funding each year. The priorities identified in the 2011 Community Profile include:
Priority One: Enhance education and outreach of breast-self awareness message to increase early detection.
Priority Two: Improve access to direct care and the continuum of care.
Priority Three: Improve quality of life through survivorship support programs.
The Community Profile is a snapshot of the community at a specific point in time. It is specifically looking at breast health and breast cancer. The profile includes:
