 SCREENING AND DIAGNOSTIC PROJECTS
Providing access to mammograms and diagnostic testing.

76th Rummage Sale Mammograms
This program will offer 40 mammograms to uninsured, underinsured and minority women through a mobile mammography unit with the goal of increasing early detection of breast cancer in the underserved community.

Breast Health for Life: Ensuring Low-Income Uninsured Women’s Access to Screening
This program will provide 600 screening mammograms, 84 diagnostic mammograms and 84 ultrasounds to low-income, uninsured patients of predominantly minority race and ethnicity. A bilingual Patient Navigator will provide supportive care throughout the screening cycle. Partnerships with local healthcare institutions will link patients in need of advanced diagnostics and breast cancer treatment with affordable care.

Breast Health Screening for North Phoenix Uninsured and Underinsured
Desert Mission’s Community Health Center provides primary care medical services to low income uninsured and underinsured children and families.  The clinic anticipates serving 550 women. More than 90 percent of these women will be minority, primarily Hispanic heritage. In partnership with John C. Lincoln Breast Health and Research Center, 160 underserved women over the age of 40 will receive free or low-cost screening mammograms and 20 will receive free or low-cost diagnostic workups, including diagnostic mammograms, ultrasounds and biopsies.

CGRMC Foundation Screening and Diagnostic Mammography Program
This program will provide screening and mammograms for 300 medically uninsured or underinsured women, ages 40 and older, living in Pinal County.  Additional diagnostic testing will be provided for those with abnormal screening results through a partnership with Central Arizona Breast Center.  A dedicated Breast Care Coordinator will assist patients needing additional procedures by arranging appointments and providing emotional support throughout the process.

Fighting Breast Cancer Together
The purpose of the program is to increase screening rates and breast health education among the underserved population. This project will provide 200 free screening mammograms through seven to eight community screening events in West Central Phoenix to uninsured women who have never had a screening mammogram. Two bilingual Promotoras will provide community outreach and breast health education workshops throughout the year. All staff is bilingual. Follow up resources and referral support will be provided for any abnormal mammograms.

Gila River Indian Community Screening and Diagnostic Mammography
This program will provide outreach, education and screening for uninsured/underinsured Native American women. Increase community awareness through education on breast cancer and screening for 110 uninsured/708 insured Native American women age 40+. Mobile mammography screening will be utilized to eliminate transportation barriers.

La Paz Hospital Outreach Mammography Improvement Project
La Paz Regional Hospital and its clinics in Bouse, Salome and Quartzsite will provide 175 mammograms to eligible Hispanic farm workers, Native Americans and rural residents of the Arizona "outback." The objectives of this program are two-fold, to educate women on the need for annual mammogram screenings, and give those at risk the tool of early detection and follow-up.

La Sagrada Familia Health Fair
Through an annual health fair, this project will provide 80 screening mammograms to medically underserved, uninsured women over the age of 40. Mobile mammography will be provided to residents of the community where 95 percent of the residents live below the federal poverty limit and are minority.

Mobile Mammography Program
By targeting Latina women, many of whom have no health insurance and lack access to mammograms, this project will address gaps and disparities in breast health care. Staffing the project with bilingual/bicultural staff and providing free mammograms removes the language, cultural and financial barriers. A mobile mammography unit will be used to provide 120 no-cost screenings to priority demographics providing vital health screenings that will potentially save lives through early detection.

Screening Mammogram Project
This project is a fundamental component of Maricopa County Department of Public Health Well Women HealthCheck Program (MCWWHP). This life saving project provides a last resort resource for women living in Maricopa County that need a screening mammogram for breast cancer.  This project will provide approximately 530 screening mammograms to women who do not qualify for other programs including the Well Woman HealthCheck Program.

Screening and Diagnostic Services
The Community Health Center of Yavapai (CHCY) addresses the needs of low-income, uninsured, underinsured and underserved individuals.  Funding for this project will provide 200 screening mammograms, 100 follow-up diagnostic and/or breast ultrasounds and nine breast needle biopsy procedures free of change to CHCY patients, 104 hours of patient navigation services in English or Spanish by the care coordinator. The patient navigator assists patients to ensure continuity of care, appropriate referral and follow up.

 TREATMENT PROJECTS
Improving access to direct care for breast cancer.  

Breast Health for Life: Access to Breast Cancer Treatment in Collaboration with Mayo Clinic in Arizona
Mountain Park Health Center in collaboration with Mayo Clinic in Arizona, will provide eight low-income, uninsured women screened through Mountain Park Health Center and biopsied through Mayo Clinic in Arizona's charitable care program with breast cancer treatment per individual treatment plan. A full time, bilingual Breast Cancer Screening Patient Navigator, will assist patients throughout the screening, diagnostics, pre and post treatment process, and provide support in accessing care at Mayo Clinic in Arizona.

Cycle of Care Project
This life saving project provides a last resort resource for women living in Maricopa County who have breast cancer. Requested funding will allow MCWWHP to provide approximately 75 to 130 treatment procedures, including higher cost cancer treatments such as chemotherapy, radiation, lumpectomy, or mastectomy for women who otherwise would go without cancer treatment procedures.

Patient Care Cost Assistance
This project is a safety net for underserved populations in Cocoinino, Navajo, Mohave, Yavapai and Apache Counties including Hispanic, Native American, uninsured and under-insured women linking them to timely, affordable care and has established a treatment network of providers accepting reduced rates, stretching funds. This program provides direct assistance to over 90 women and enables continued outreach to the undocumented population in Coconino and Navajo counties resulting in 300+ mammograms annually.

 EDUCATION PROJECTS
Enhance education and outreach of breast self-awareness message to increase early education.

Asian Pacific Women's Health Initiative
The focus of this initiative has been to educate Asian American and Pacific Islander women in Maricopa County regarding breast health and to assist these women in accessing mammography services. For the project year, it is projected that 550 women will receive comprehensive breast cancer prevention education through lay worker outreach, one-on-one counseling, expert led workshops, and 100 women will be successfully referred for mammography.

Navajo Nation Breast & Cervical Cancer Prevention Program
This program will increase breast health and breast education to Navajo women. The education effort will be composed of direct education contacts and mass media announcements.  A bilingual Health Educator will attend community events and provide presentations to community groups and organizations. Designed to inform women about breast health and resources available, such as screening services, this project is expected to reach 4,000 women.

 CONTINUUM OF CARE PROJECTS
Supporting a woman or man's complete journey from knowledge to screening to diagnosis to treatment and through recovery.

Continuity of Breast Care for the Uninsured and Underinsured
The Continuum of Care begins with outreach and education and includes assistance in applying for public health benefits such as AHCCCS (Medicaid) and the Well Woman HealthCheck Program. This program walks individuals through the breast cancer journey with the highest quality breast care services, regardless of their financial or insurance status. Program will outreach to educate 5,000 women. Screening mammograms will be provided to 150 uninsured and underinsured individuals, 100 diagnostic work-ups, 20 biopsies. For up to six individuals who are found to have breast cancer, the patient navigator will coordinate treatment services such as surgery, radiation, lymphatic drainage, pharmaceuticals and/or chemotherapy. Wrap around services for those battling breast cancer and for survivors will include monthly support groups and classes, free wigs and prosthetics, and a multi-media library.

Laura Dreier Breast Center’s Reaching Out Continuum of Care Program
The aims of this project are to address breast health needs of underserved and uninsured women in metropolitan Phoenix. The program will provide education, community outreach, screening exams, diagnostic services, breast cancer treatment, and survivor support services in a caring and culturally sensitive environment.  Women will receive a variety of free services at the Laura Dreier Breast Center at Banner Good Samaritan Medical Center.  The program will employ Nurse Navigators to assist patients throughout the continuum of care.  Patients will also be assisted with applications for financial assistance, or other means of funding for eligible patients.
